[kdewebdev-webdeveloper] Form Problem

Hello Folks,

I created the attached simple form in Quanta and uploaded it to the 10M 
space my ISP provides for my web page. I opened it in my browser and 
filled it in then 'submitted' it. My email client's message composer 
popped up and I hit the 'send' button. I then picked up the message from 
my email account and there was no attachment with the form values.

Can somebody tell me what is wrong with my form please, why the data 
from the form is not attached. I am a complete novice with forms.
